Polypectomy is the endoscopic procedure through which abnormal tissue is removed from the large intestine. This abnormal tissue can transform over time into colon cancer. Its removal prevents this transformation and avoids the need for surgery. Not all colon polyps have this risk of malignant transformation, the decision to remove them should be taken together with the endoscopic doctor, sometimes after obtaining a histopathological result (microscopic examination of a polyp fragment obtained during colonoscopy). Most of the time, colon polyps do not cause symptoms, so just occasionally, in the case of large polyps, removing them will also solve symptoms such as stool bleeding, abdominal pain, or intestinal transit disorders.
Complications are rare, but no medical procedure is completely free of risk.
Before the procedure, the doctor:
• Can perform a clinical examination; • Asks about your medication for chronic diseases (anticoagulants like Trombostop or antiplatelet such as aspirin or clopidogrel increase the risk of bleeding after polypectomy); • Can ask for blood tests (blood count, coagulation tests, etc.).
The success of the polypectomy depends on the quality of the intestine preparation (cleaning it with the physician-recommended solutions, along with the clear hydric regime from the pre-examination day).
A flexible tube with a recording camera at its distal end is inserted through the anus to the end of the large intestine. During the examination, the intestine will be dilated by the air introduced through the endoscope. The physician locates the polyp and removes it either by cold cutting or by using electrical current (electro-cutting). The same current can be used for the cauterization of the remaining wound or for stopping postpolipectomy bleeding. The resected polyp will be recovered from the intestine and sent for full microscopic analysis. If removal of the polyp is not possible via the endoscopic route, it is advisable to mark its position for subsequent surgical removal (through a minimally invasive, laparoscopic procedure). The duration of the procedure is 30-60 minutes, depending on the case.
The polypectomy is not more painful than the diagnostic endoscopy. The discomfort is caused by the air distension of the colon and by the pressure the device exerts on the walls of the intestine. Sometimes it is useful to use a superficial sedative in order to relieve discomfort.
