Superior digestive endoscopy is an investigation procedure by which the physician can explore the esophagus, stomach and duodenum using the endoscope, a thin and flexible instrument with an optical device. The endoscope is inserted through the oral cavity and moves slightly via the throat, investigating the esophagus, stomach, and then the duodenum.
It is a painless, short-term (3-5 minutes) investigation which does not affect breathing, and the discomfort of the investigation is given by the perception of the tube in the throat; local anesthesia with xiline helps calm the unpleasant sensation.
In order to perform a superior digestive endoscopy, the stomach should be completely empty-fast for 8 hours. Medications and liquids can not be given either. Small water inlets are allowed up to 2-3 hours before.
When the patient visits for the investigation, the doctor discusses the symptoms requiring the investigation, the medical aspects of interest and explains the procedure. Then the patient reads and signs the consent form for the investigation.
Before starting the investigation, a xiline spray is used for local anesthesia in the throat so as to relieve the sensation of nausea and local reflexes. Then a mouthpiece is inserted in order to protect the endoscope from bites, and the endoscope insertion tube is introduced through the piece and advances to the duodenum. During exploration, air is introduced and a slight feeling of bloating may occur. The patient can also perceive the movement of the endoscopic tube into the abdomen, usually without feeling any pain. The examination is performed both during the insertion of the tube and especially during its withdrawal, when the stomach is in distension because of the air.

If changes are observed, biopsy can be taken. Biopsy does not cause pain to the patient. Also, biopsy can be taken for rapid Helicobacter pylori test if there is an appearance of gastritis or gastric or duodenal ulcer. The Helicobacter Pylori test is readable for up to one hour, but most often it is set at 5 minutes. The rapid endoscopic test has the best accuracy for the diagnosis of the bacteria and enables prescribing the correct treatment. It is recommended - if possible - for the antisecretory treatment (Controloc, Omaz, Omeran, Nexium, Lanz, etc.) to be stopped 1-2 weeks before performing the endoscopic test.
If the biopsy is performed in suspicion of malignancy, the patient should not worry about a potential seed and dissemination of the tumor because the biopsy is taken "from the inside" without crossing other tissues and is absolutely necessary for proper treatment.
It is very important for the patient to be aware that the procedure and, if necessary, the biopsy are absolutely necessary to accurately determine the diagnosis and implicitly an appropriate treatment and not just a symptomatic treatment.
The entire investigation takes 3-5 minutes. The duration of the investigation is shorter and image is better when the patient co-operates and stays relaxed. Biopsy can prolong the investigation for 1-2 minutes and cause a slight local bleeding that stops shortly, most often before the endoscope withdrawal. It is recommended for the patient not to consume hot liquids or foods that stimulate gastric acid (coffee, hot spices, citrus) after the biopsy.
The superior digestive endoscopy represents a simple, minimal risk analysis unless therapeutic procedures (resection of polyps, hemostasis, etc.) are performed. In general, complications of upper gastrointestinal endoscopy are rare. Overall, the complication rate ranges between 0.02% and 0.2% (less than 1 in 500 cases). Possible complications are: bleeding (0.15%), perforation (rarely reported in superior digestive endoscopy for diagnosis purposes), infectious complications in patients with cardiac valvular disease (endocarditis risk estimated in 1 out of 5 to 10 million patients).
